C. Your contact information, including:

2. Counter-Notification

If you believe that material you posted on the site was removed or access was disabled by mistake or misidentification, you may file a counter-notification. To be effective, your counter-notice must include:

A. Identification of the material that has been removed or to which access has been disabled and the location at which the material appeared before it was removed or disabled.

B. Your contact information, including your full name, mailing address, telephone number, and email address.

C. A statement under penalty of perjury that you have a good faith belief that the material was removed or disabled as a result of mistake or misidentification.

D. A statement that you consent to the jurisdiction of the Federal District Court for the judicial district in which your address is located (or if outside the United States, for any judicial district in which the service provider may be found), and that you will accept service of process from the person who provided the original DMCA notification or an agent of such person.

E. Your physical or electronic signature.

Upon receipt of a valid counter-notification, we will forward it to the original complainant. The original complainant will have 10 business days to notify us that they have filed a court action relating to the material. If we do not receive such notification, we may replace or cease disabling access to the material at our discretion.

3. Repeat Infringers

It is our policy, in appropriate circumstances and at our discretion, to disable or terminate the accounts of users, contributors, or account holders who are determined to be repeat infringers of copyright.
